Paper Work


Different agencies and different countries require small variations in your paperwork. This is a fairly standard list and is a good place to get started.
  • Photographs of the front & back of your house, rooms in your house, yourselves
  • Birth Certificate
  • Marriage Certificate
  • Divorce Decrees (if applicable)
  • Home Study-Completed by a licensed agency/social worker in your state of residence
  • Physical Examination
  • Financial Statement (each agency will have a form for this- information needed: Salaries, pensions, bonuses, other income, bank accounts, stocks, bonds, real estate, IRA’s, value of home, furnishings and personal property, loans such as mortgage, credit cards, education, appliances, etc)
  • 3- Letters of Reference (non-family members)
  • Letters of Employment-indicating length of time employed and current salary
  • Passport (and passport copies)
  • Police Clearance
  • Post Placement Agreement
  • Statement from your employer or insurance carrier stating that the child will be covered by medical insurance.
  • Copies of Past Three Years Tax Returns
  • Copy of your state law (regarding adherence to US laws and confirming that the child will have the same rights and privileges as US citizens and natural born children).
