Letters of Employment-indicating length of time employed and current salary

Passport (and passport copies)

Police Clearance

Post Placement Agreement

Statement from your employer or insurance carrier stating that the child will be covered by medical insurance.

Copies of Past Three Years Tax Returns

Copy of your state law (regarding adherence to US laws and confirming that the child will have the same rights and privileges as US citizens and natural born children).
